Fees and Payment
Individual, Marriage, Family Counseling/Coaching
Fees for services are determined by the length and type of service, and by credentials: 50 minute session ($180). Group therapy and other group services are billed per session. Phone consultation and other services are billed on the basis of time and materials involved, consistent with the clinical hourly rate.
The preparation of documentation and letters or forms will be billed at the clinical rate of $180 per hour.
Diagnostic assessments, mediation and other services may vary in duration and sometimes are divided into more than one appointment. Feel free to discuss these fees with your me.
Payment is the responsibility of the client or the parent if the client is a minor. Please note that by receiving services, you are agreeing to pay for them. In cases of shared custody arrangements, the individual who agrees to be the person responsible for payment is the billable party.
Payment is due at the time of service unless other arrangements have been agreed upon in advance. Payment may be made by check (payable to Restore, LLC), cash, or credit card.
Premarital Counseling/Coaching
Premarital counseling involves a customized couples assessment and counseling interventions to identify and improve strength and growth areas in the relationship. This assessment and counseling program meets the diverse needs of clients and is customized for you. Based on the assessment, you will receive counseling sessions in which you discuss and understand these strength and growth areas as you are taught proven relationship skills.
Online Assessment ($35). Reduced rate sessions, $150 per hour. Typically the first session is a 2 hour session. Then we can customize the number and timing of sessions based on the couple’s input and needs.
Life Coaching
Life Coaching is not counseling but an ongoing professional relationship which focuses on forward movement to produce positive results in life, career, and business.
Through focused conversation and the coaching process, one will deepen their learning, improve their performance, and enhance their quality of life. Results are a matter of intentions, choices and actions. Through the support of the coach and the coaching process, this can be realized.
Sessions are billed at $180 per 50 min session.
Insurance
Many of our clients prefer not to use insurance as payment, because insurance companies require a diagnosis that has the potential of being shared with employers. (Insurance companies require a diagnosis, prognosis, and progress reports). Having said that, many of our clients are able to use insurance benefits to get reimbursement. Clients can call their insurance companies and find out about their eligibility for reimbursement. Insurance companies will ask you for the billing codes (also called CPT codes) for your sessions.
Please check with your insurance company directly to determine your benefits. We can provide a receipt (called a Superbill: A Superbill is a receipt for sessions with an out-of-network counselor allowing the client to submit to their health insurance. A Superbill may be requested by a client of a mental health professional, when the client pays out-of-pocket for the qualified expenses. Reimbursement will be determined by the individual healthcare policy at the time the claim is received), which you can then submit to your insurance carrier for direct reimbursement.
Telephone Consultations
Telephone consultations between sessions will be billed on a prorated basis according to your clinician’s fee structure. There is no charge for a brief phone call under ten minutes in duration. When calling please leave the times that you can be reached at the numbers you leave.
If you want your therapist to have phone contact with family, friends or other professionals you will be asked to sign a document authorizing this contact in advance.
Release/Duplication of Records
The charge for copying records is $50, which includes postage and handling. A charge for releasing records under 20 pages by fax is $20.00.
Appointments and Cancellations
I will do my best to be punctual for your appointment and ask that you be punctual as well. Even if you are late for your appointment, I will have to end the appointment at the scheduled time. This is in order to respect the schedules of other clients and of your counselor.
If you need to cancel or reschedule an appointment, please contact me at least 24 business hours in advance (by Friday for a Monday appointment) so that the time may be made available to someone else. Failure to give 24 business hours notification may result in a fee of $90. Failure to come in for an appointment will be treated as a failure to give adequate notice for a cancellation, and therefore will be charged as a full counseling session.
